Given that we talked about a previous provider putting things in the chart that hadn't been discussed, I found it particularly disrespectful that he put things in the chart that hadn't been discussed (or maybe I slept through it).

The whole office deserves to be recognized for their exceptional service and great patient care, I’ve never seen anyone who is not happy with going to their office, they definitely know how to make sure the patient is taken care of. If they could be given an award for their compassion and kindness while caring for their patients, they definitely would deserve it.
