Application Process
Applications for the CHRISTUS Santa Rosa Primary Care Fellowship Program are accepted beginning July 15 of each year. Interviews are held from September through December of each year.
Our application deadline is October 1.
Eligibility Criteria
All applicants must be from a residency program accredited by the Accreditation Council for Graduate Medical Education in either Family Medicine, Internal Medicine, or Pediatrics. Applicants must be eligible to obtain a permanent license to practice medicine in the state of Texas.
Our program participates in the National Residency matching Program. All Applicants must apply through Electronic Residency Application Service (ERAS®).

If you meet the eligibility criteria above, and successfully submit each of the items below, you will be considered for an interview with the Primary Care Sports Medicine Fellowship.

- Personal Statement
- State License or Permit Information
- Three reference letters (one from current Program Director)
- USMLE Scores or Comlex Scores
- Medical School Diploma
- Medical School transcripts
